We are currently accepting qualified applications for a RN Hospice Clinical Manager to support one of our Hospice and Home clients in beautiful Modesto, CA.
Job Summary:
Supervises and directs the skilled nursing and other therapeutic services provided by Optimal Hospice.  Ensures quality and safe delivery of hospice services in accordance with hospice standards and applicable laws and regulations. Ability to demonstrate the knowledge and skills necessary to provide care, based on physical, psychological, educational, safety and related criteria, appropriate to the age of the patients served in his/her assigned service area. The skills and knowledge needed to provide such care may be gained through education, training or experience.
  • Must possess a current California State Registered Nurse license.
  • A nurse supervisor shall be a registered nurse with two (2) years’ experience within the last five (5) years in a hospice agency, primary care clinic, or health facility.
  • Must have a valid driver’s license in the State of California and an insured automobile in good operating condition.
  • Current CPR certification
  • Must be able to manage multiple priorities and possess effective time management and organizational skills.
  • Must be able to effectively problem solve employee and client/patient issues
  • Overnight travel will be required at times
  • Monitors and directs the staffing needs of the agency by employing, counseling, terminating, and directing staff as appropriate.
  • Make assignments that consider the needs of the patient.Adjusts assignments and prioritizes in response to a change in workload as necessary.
  • Promotes a safe environment through compliance with Standard Precautions, Bloodborne Pathogens, and use of proper body mechanics.
  • Effectively uses Quality Management concepts for developing, managing, evaluating, and maintaining standards of care.
  • Assures compliance with ACHC Standards, State and Federal regulations.Takes action when standards are not met.
  • Holds staff accountable for documentation of nursing practice for assigned patients or case-loads.
  • Assures that departmental policies and procedures are current with changing standards of care.
  • Plans unit objectives with the assistance of Administrator, based on cohesiveness with long range plans and coordination of goals and objectives of other departments.
  • Plans, prepares, and implements quality management with the assistance of Administrator/Director of Patient Care Services.
  • Regularly evaluates the care given by each staff member against performance criteria or competency standards and outlines strengths and areas needing improvement, addressing specific behaviors that are conducive/detrimental to the established goals.
  • Provides written documentation and follow-up when employees are counseled about recurring or significant problems.
  • Establishes relationships that build networks to promote professional growth.Is responsible and accountable for maintaining a professional level of nursing practice.
  • Evaluates own performance and seeks validation resulting in a positive change.This includes maintaining and continuing to develop clinical and management expertise.
  • Reports to Administrator pertinent communications and supports agency standard of excellence.Utilizes effective oral and written communication skills, as demonstrated in a purposeful, relevant, non-judgmental manner and is understood to staff and other departments in all aspects of operations.
  • Handles problems in a professional manner, seeking assistance when necessary, using established lines of communication for problem solving and appropriate conflict resolution.
