We are currently accepting qualified applications for a PHYSICAL THERAPIST to support one of our Home Health clients in beautiful Virginia Beach, VA.
Directs physical therapy treatment toward preventing disability, relieving pain, developing, improving and/or restoring muscle function and maintaining maximum performance within the client’s capabilities in accordance with the plan or care/treatment. The PT will oversee physical therapy assistants to assure appropriate implementation of the plan of care.
  • One year of physical therapy experience (acute care/home care setting) or as required by law
 Required Skills:
  • Knowledge and ability and the necessary skill and judgment to perform all procedures as outlined in the scope of practice
  • Ability to contribute as a team member and ability to work independently
  • Excellent verbal and written communication skills; strong interpersonal skills
  • Knowledge of the regulatory requirements at the state, federal and local level
Essential Functions:
Clinical Services
  • Evaluates the client’s level of function by applying diagnostic and prognostic muscle, nerve, joint and functional abilities tests
  • Evaluates the home environment for rehabilitation and makes appropriate recommendations to increase the independence of the client
  • Supplies physical therapy services in the home as prescribed by a physician
  • Administers manual therapeutic exercises; instructs, motivates and assists clients in non-manual exercises and functional activities
  • Administers traction, massage and treatments involving the application of physical agents
  • Evaluates, fits and adjusts prosthetic and orthotic devices and recommends any modifications to the physician
  • Participates in the development, review, and updating of the care plan
  • Observes, records and reports any changes in the client’s condition and response to therapy and makes
  • suggestions for changes in the plan of care to the client’s physician and Case Manager
  • Prepares clinical records (initial evaluations – including OASIS, care plans, progress notes, discharge summaries) and submits them for the client’s clinical record
  • Participates as a member of the interdisciplinary team in case conferences and in planning and evaluating the total treatment for the client and confers with interdisciplinary team members as needed
  • Advises and consults with the client/family and other agency personnel
  • Submits orders and documentation (clinical and payroll) on a timely basis and according to branch policy
  • Instructs and supervises Home Health Aides or CNAs/STNAs, as applicable, in certain phases of physical therapy services they may to the client
  • Instructs the client/family in the use and care of physical therapy devices and on client’s total physical therapy program
  • Instructs and supervises Physical Therapy Assistants in certain phases of physical therapy services they may provide to the client
  • Supervises the Physical Therapy Assistant minimally as required by applicable state law
  • Completes an on-site functional assessment, review and revision of the plan of care and assessment of utilization of community resources as required by client
  • Accessible to the Physical Therapy Assistant when a Physical Therapy Assistant is providing treatment to a client
  • Participate in the marketing efforts of the office as needed
  • Participates in community relations activities as requested
  • Promotes the agency through positive representation and communication of its services
Coordination of Services:
  • Communicates effectively with all team members
  • Attends case conferences as requested
  • Assists with quarterly reviews, if requested
  • Contributes to the effectiveness of case management and coordination of services through open communication
Financial Management:
  • Understands and contributes to the financial management of cases within guidelines of the payer and client rules
  • Understands and complies with parameters of budget relating to expenses
Policies, Procedures, and Documentation:
  • Provides care as outlined in the client’s plan of care and documents appropriately in the record
  • Follows all company policies and procedures
  • Submits all documentation (clinical and payroll) timely and per branch policy (at least weekly)
  • Assures and completes accurate documentation with all clinical data including but not limited to admission through discharge, updates, OASIS, HCFA 485, physician’s orders, skilled nursing visits and changes in conditions
  • Is punctual for client visits and reports to work as scheduled
  • Is flexible and works cooperatively for agency needs
  • Quality Improvement
  • Understands the concept of continuous quality improvement
  • Applies the concepts of quality improvement to all work practices
  • Is knowledgeable of the agency’s QI goals and participates as directed
  • Graduate of an accredited physical therapy program
  • Current valid license as a physical therapist by the state(s) of practice
  • Current CPR certification
  • Valid Driver’s license and motor vehicle insurance
