We are currently accepting qualified applications for a RN Home Health Clinical Mentor to support one of our Hospice and Home clients in beautiful Bakersfield, CA.
 
 
Job Summary:
Responsible for assisting in the development of clinical standards and field implementation by providing on-site education and mentoring to the clinical staff in each office. The home health clinical mentor works closely with the clinical managers to provide feedback on the status of new and existing employees. Identifies opportunities for improvement in the orientation program or process, and participates in the development and implementation of innovative teaching tools and programs designed to raise the knowledge base of our clinicians in order to bring the highest value to our patients.
 
Qualifications:
  • Must possess a current California State Registered Nurse license.
  • Must have a minimum of 3 years’ experience in home health nursing.
  • Must have good interpersonal skills and enjoy working in a team environment.
  • Experience using a healthcare related software application.
  • Must have the ability to teach in a constructive manner, and the desire to work with the clinical team to assure high quality clinical standards.
  • Must be certified, or working towards a Certificate for OASIS Specialist-Clinical (COS-C).
 
Job Duties:
  • Provide education, training and support in the use of Allscripts, in the classroom and in the field and acts as a super user and resource to all Allscripts users.
  • Acts as a clinical resource to management and clinical staff.
  • Responsible for the implementation of the clinical support and mentoring program for all field staff, and works in the field with clinicians to assure that our clients’ clinical standards are being met.
  • Participates in the education and training of new and existing hospice clinicians.
  • Provides feedback to the clinical managers regarding ongoing development of their staff.
  • Develop skills lab modules and training sessions for ongoing improvement of existing staff’s clinical skills.
  • Review clinical documentation and evaluate areas in need of improvement with management.
  • Instruct field clinicians on improvement techniques to meet documentation regulations.
  • Work in conjunction with the QI Coordinator reviewing focus areas and implementation of education needed to meet performance improvement projects.
